Job Description:
A company is looking for a Telephonic Care Navigator who will
assist patients in navigating the healthcare system and connecting
them to necessary resources.
Key Responsibilities:
Conducts telephonic outreach to members and educates them on care
gaps
Links members to community resources and ensures access to primary
care providers
Maintains schedule adherence while providing excellent customer
service
Required Qualifications:
High School Diploma / GED or equivalent work experience
Minimum of 1 year of healthcare, insurance, or community outreach
experience
1 year of call center or telephonic customer service experience
Experience using Microsoft Office applications
Ability to work from home with high-speed internet access
