Job Description:
A company is looking for a Clinical Case Manager Behavioral
Health DSNP.
Key Responsibilities
Conduct comprehensive assessments of members' needs and eligibility
for services
Perform crisis intervention and coordinate care with clinical
providers
Develop care plans utilizing clinical judgment to address member
issues and improve outcomes
Required Qualifications
3 years of direct clinical practice experience post-master's
degree
Knowledge of mental health and substance abuse disorders
Experience using Motivational Interviewing and technological
skills
Valid unrestricted independent professional behavioral health
clinical license
Must be available to work Monday through Friday, 8:00am to
5:00pm
